cHALLENGE:

Liverpool City Council wanted to transform its Adult Social Care pathways to embed a preventative, strengths-based model of social care, improving outcomes and experience for both residents and staff.  ​


To redesign pathways into Adult Social Care, to ensure opportunities to maximise independence, with the right resources in the right places to enable residents to receive proportionate support to their needs. ​

IMPACT:

Completing a Demand Analysis Report to identify areas of comparative strength for Liverpool, and to identify areas that required a more detailed focus as part of any transformation activity  ​

Reviewing and redesigning service pathways to focus on prevention and independence wherever possible – including a redesign of the Front Door with an emphasis on increased self-service and the introduction of an MDT approach to improve the take-up of short term support​

Undertaking cost-benefit analysis for a preventative reablement offer, which indicated that Liverpool had the capacity to introduce this offer and it would unlock benefits of £660k in cost avoidance, as well as improving independence for residents​

Creating a high-level strategic approach to commissioning for the service, to enable commissioning to become more strategic and focused on future needs of residents​

Developing a risk-led prioritisation dashboard for planned Annual Reviews to support staff to manage their workloads, ensuring residents receive reviews when they are needed ​

Throughout this work, working in partnership with LCC transformation and service colleagues, ensuring knowledge transfer so the service can have a sustainable approach to transformation ​

 
Through deep collaboration with the Council, this provided Liverpool with:​

  • A community reablement offer which would unlock up to £660k in avoided costs and improved outcomes for residents.  ​

  • A new Front Door TOM, which is due to be fully implemented by March 24, supporting the delivery of £13.4m savings over three years​
